Overseas the tapered are known as "hillside" washers.Some proper tapered shims instead of a stack of washers would work a fair bit better in that taper flange I beam.

Throw the spring washer - its just another flat washer when tightened - and the crappy wire ones simply open - as you found already. You've defeated any theoretical ability of it to stop the bolt coming loose with the flat washer anyway. If you want it to stay done up against vibration, a nordlock washer under the bolt head without that additional flat washer, and another under the nut (no flat washer there either) - if its going into a nut plate or captive nut that can't rotate, just the nordlock under the head will do.
