I don't know if @Parm is able to comment know he's I believe out of the game but I don't really think the HSE are interested or even have the resources to get involved before something happens. Some industries are so niche they wouldn't have a clue. In the event of an accident they anyway I think would call in an expert witness. It's how companies get away with shoddy practices. The reality is they get away with it for most of the time. Any employee who flags issues up is seen as a troublemaker and will likely find themselves out of a job.

Does the HSAW 1974 trump an employees contractual obligations? If a company won't listen and you're prohibited from airing on social media where do you go?

...If you take it further in industry you gradually evolve towards the methodology pilots use on commercial flights, procedure and everything is double checked using a check list, not matter how many times they have done it. By doing this everyone does a task the same way too, so swapping team members between shifts for example shouldnt be an issue as in theory all folk do the task the same way.

'Lists' were the latest thing for HSE just as I left the oil industry. Our employer binned our previous safety system, as they did every ten years or so, and the new one was lists. Everything was on a list, we had lists of lists, and lists of lists of lists. Everybody had to fill in a list of things to do, for every operation.

The captain of our ship told us all about it in the last safety meeting of the trip, and we all just sat there speechless, looking at each other in amazement.

Years of carefully written out and established job procedures were binned, and they would all have to be done again in list form.

Luckily I got laid off before the next rotation.

So I work with chaps who's primary defence is "we've always done it like that".

These people need checklists as they a missing many many issues.

We had an 'old boys club' who were a bit like that, any new blood on board was verbally beaten down at any opportunity. New hires were treated like crap, anything they suggested was instantly dismissed etc.

I basically worked on my own, just me doing my job, maintaining massive air compressors, so I didn't get involved with any of it. Apart from my last two years offshore, when things changed and I was forced into a different job role. That was when I decided I wanted to get out, and I stopped 'participating' in the company promotion agenda.
