I don't know if @Parm is able to comment know he's I believe out of the game but I don't really think the HSE are interested or even have the resources to get involved before something happens. Some industries are so niche they wouldn't have a clue. In the event of an accident they anyway I think would call in an expert witness. It's how companies get away with shoddy practices. The reality is they get away with it for most of the time. Any employee who flags issues up is seen as a troublemaker and will likely find themselves out of a job.
