only one of the cracks are actually at the bolt hole. The others are around the perimeter of the rudder box, not saying what you have noted is incorrect but i firmly believe that the poor layup with multiple voids and the actual lack of material in such a high stress part of the hull are the main reasons for the failure.Looks like the cracks have manifested from the holes, will this happen again once laminated.
as for preventing a reoccurrence , I have ground the cracks too allow a better bond and the whole area will be built up from the nominal 9mm thickness to nearer 18mm, hopefully this will prevent it happening again at my repair point.
been working with these sort of craft for over 35 years now and this is the worst i have encountered.
last one i repaired with similar damage had been dropped from about 6 feet onto concrete !
