Made a screw, because a 5.5mm X40tpi is not readily available, took me about thirty mins but a lifetime to accumulate all of the tooling to make such a thing, 4mm hex:

As you can see, I had to drill the original out.
Snap, but 3/8" x 40t
and two flats filed for a metric spanner (I used round stock as had no suitable hex bar)
I bought a die from Tracy Tools to do it that way, they seem quite good for odd imperial taps and dies
Did you make it from a larger bought in screw? otherwise, how did you cut out the hex?
