For realy confined spaces I use a HT caphead bolt + 2 well fitted washers all well greased (flange type inserts,) For c/sunk type you need to turn a thick washer with a small cone .

Rivnuts if not installed correctly can spin, and are not intended for continual use or high torque . Where this is likely these should be used Hexagonal type,
If you think the bolt will be installed/removed then a couple of tacks with the TIG should hold it fast
