That appears to be a changegear lathe with no gearbox so you'll need every size listed on that plate. Those splined gears tend to be expensive so buy wisely. Run the carriage up so the tool is about 2-3" from the chuck, note the location of the half nute then move the carriage away and inspect the leadscrew. The threads are likely to be very visibly worn at that spot.
