It’s very obviously clear with abundance that you have absolutely no experience in such mattersIMO the whole workmanship is questionable. The 'welding' is clearly substandard and the strength of the skid inadequate at the load points. Supposing that I beam hadn't just bent, but torn? I've seen it a thousand times in demolition even used it to advantage more than once. Your sling is then pulling around a ragged edge not just a plate with a bend in it. A toe plate welded across the beam at the load point would probably be all it needed and those legs should have web stiffeners above them on the load path (but not welded by the same guy that glued the legs on).
Any lift supervisor who lifted that load twice deserves to be sacked and their ticket pulled IMO.
I was a little worried posting pics of my own welding on my trike project, they're a little wobbly, like my hands, but it'll never have to hold 60 tons, so I shall sleep soundly.Anyone who looks at that weld can see it's undercut, not penetrated properly and not even along the complete joint. I can see 5 defects (circled) before we even get to the holes in it. Deficient is an understatement and I am being more than generous there. If I had turned out anything like that in my Crane Fruehauf days I would have been told to go and never come back. It's a disgrace.

The welding is bad and the frame is equally as bad. I dropped it off safely and we didn’t lift it again thankfully.
Also the shipping documents showed where to lift it from. Which was less than ideal.
