the jaws themselves are pretty hard so id say your theory isnt far out, the slots must of wore or pulled slightly.I have a bell-mouthed Pratt chuck here too and I was surprised to find that it was the chuck at fault and the jaws were perfect. I had previously assumed that knackered chucks would be down to the jaws wearing but I later bought a similar 3-jaw that was missing the inside jaws, so I tried the jaws from the knackered chuck and found them to fit and grip really nicely. The only thing I can attribute it to is someone overloading the original chuck with something gripped right on the jaw tips and stretching the jaw guides or something like that.
i see some grinding the jaws back true again,
i watched a few vids of lads grinding the 3 jaw chucks, i seen mr pete go about it all fancy and other lads just grinding them out with no tension on the scroll and in the end both ways came out as accurate.
