They are like a thick waxed paper to form the cone - the trick is to ensure the groundworker waggles the bolts at the end of the day when the concrete has started to go off.
In my old job, me and the boss would go round after the concrete had been poured, with sections of ply that matched the corresponding steel base plates & holes to ensure all the studs would line up.
