I didnt spot that the cap was indeed a cap untill I had it pressed quite forcefully half way through my pulleyI usually heat the top of the pulley. The cap isn't too tight. At a push you could unscrew the nipple, which is an oil nipple, and screw something in to help easy it out. I can't recall the thread at the moment, but probably BSF.
Usually I heat the pulley, after removing the grub screws (check that they haven't been doubled up), and then prise the pulley off using tyre levers and packing pieces as necessary.
You might have a bent shaft if there's no burring or muck in the teeth. Unlikely but I suppose that damage to the teeth is a possibility. But these are very well engineered and I've used mine as a press on more than one occasion.
