Wow - not good!
I wonder how someone could have messed up that good for it to break loose and take off. It must have thread direction that locks it and was there not a shroud or guard in the way or maybe that was removed. Strange one that it could be possible in the first instance....

That's a road saw blade 1" hole. The flange is not attached so it's either the retaining bolt has sheared or more likely just come undone. Typically it'll be a 65HP or more engine driving it. They have pins in the flange that prevent the saw from tightening the bolt too much and these often shear especially if there's a lot of hours on the machine. The LH bolt is LH threaded so it cannot un-wind itself in use but I've seen them rattle loose with no load on them. A bit of grit under the flange and it'll soon be rattling away.
Sobering seeing it travel like that and how far it's embedded in the door pillar. I know of only one fatality in the UK it was a track mounted one cutting horizontally at about waist height and the output shaft sheared. The guard didn't stop it and it killed the operator.

Taking an ash tree down on my border with my neighbour. His side was an old galv water tank with a couple of inches of 22mm copper pointing up.
I was shifting a bough that had come down and managed to trap my pinky between it and the 22mm pipe stub. Lucky I had gloves on. Black both sides. Fitting a cat flap today I caught it and the nail started to lift:
