I think if we are going to look for facts it would probably read, “insecure load falls from trailer” I’ve been wrong in the past but looking closely I wouldn’t be moving that with what only appears to be two chains on it. They may have been rushed out of the way but regardless.

If you ever saw a long piece of concrete that had been dropped you would know right away. Sometimes in demolition you'll pick up a long thin piece of concrete and drop it deliberately especially if it's 60's or 70's era stuff it just disintegrates away from the rebar. Drop the part, grab the rebar and give it a good old shake then if you're lucky you can just dump te bare rebar cage in the metal bin.

Apart from tat there are no lifting accessories evident. They would be trapped under the load.
