Condensed Version. It was a file that was left in a pipe line And caused lots of damage ( repair work down time etc). They (ICI) had it in a display cabinet.

Many moons ago when god was a boy we were working on a twelve-mile-long pipeline, part of the job when finished was to flush it out courtesy of five fire engines all joined up on a big manifold. It took about three weeks to fill off the mains and the brigade were there just for the flush, when they started pumping there were boxes of welding rods, helmets, chipping hammers, boots, flasks, packed lunches,dead foxes/rabbits, lumps of wood and just about everything in between flying out of the end of the pipe. And lots of water

Seen Xrays of some pipe line blockages you would not believe the cause. OXY bottles, scaffold batons, one of a actuator valve (large) wouldn't close properly, Cause was a concrete fence post jammed in it.

I dont think so, more like a lack of communication. Ours was a very deep/wide trench racked back to avoid collapse, railway sleepers thrown across the trench banks and the pipe put on top of that to keep it just off the bottom of the so we could weld without digging weld pits (pipe was craned up in sections after that, sleepers pulled out and pipe lowered) With all the crew changes and different trades involved its not uncommon for things left in the end of a pipe to go unnoticed and another section lowered in.Hence the big flush

sods law bombing along the a66 today in the outer lane and a bloody great bang from back of van pulled over fast to check at 1st I though kids on the bridge had thrown something at it . nupe all ok / I looked at the back tire its as flat as a fart took it off and theres a hole in the sidewall bigger than a pencill that wont repair £125 for a new one
