Took this to my friendly mechanic today, results are inconclusive 
No codes / live data on his code reader when cranking so no way to check which sensors are doing what until it's running. Not a good design !
All looks good once it is running (apart from the baro / atmosphere sensor - we think not critical for starting).
It starts 100% fine using a sniff of easy start
I'm still not sure if the fault is fueling or electrical
His suggestions:
Turn it off on a hill, try to bump start it (done that - it fires up fine).
Check fuel pump timing using marks / locking tools (I replaced the timing belt but like-for-like and it's been starting fine, I've not set or adjusted the pump timing).
Possible tired fuel pump due to mileage (650k on the clock, unknown history). Guessing a pump rebuild could cost more than the van is worth.
