What “exactly” makes 3 pin adapters so unsafe?

I know you mean the "original picture" was in the office of a H&S inspection company but I can't help reading it as the "original" i.e. the dodgy bypassed adaptor was actually found in the H&S inspection company's office

It's mis-using them that's the problem. My Gran used to plug her iron into the light, you could get adaptors that plugged into the light socket with 2 outputs, 1 for a bulb and the other for whatever.

thats coz there where no sockets in most of the rooms then we had 4x 13a singles in the whole house !

When we moved into a house in '64 there was a 15A socket upstairs and another one downstairs

The house I grew up in had a collection of 5 & 15A sockets & lead cabling. Fused both on the live & neutral, deadly way to do it, you'd blow the neutral fuse & the circuit would remain live! Also there was one 10A 2 pin socket upstairs, never seen another one like it. The house remained like this until my mother died in '98 she refused to get it rewired & said the next owner can sort it out.

There was a lot of modernisation work required, the house was early 17 century & needed quite a bit of work to bring it up to a modern standard. My parents had lived there for 50 years doing very little to bring it into the 20th century. The family that bought it were well aware of what was needed, didn't stop them grumbling though The house has been sold at least 3 times from when we sold it, & worth a small fortune now.

What did people run from their light fittings and how many caused a fire, the connection to the plug is no more then 2 split pins that also form the prongs on the plug

We had a house like that.
