Long shot Fizzy, but do you have either another 1/2" UNC nut or bolt to work out which one may be at fault?
I have another tap which works fine but no nuts or other dies in 1/2 UNC. I did wonder if its a faulty die. Came from a cheap set but the fact the first 1/3 screws in ok suggests it is fine.
I used a tailstock die holder but running the die down by hand it seems a bit uneven.

Crest looks quite flat on that bolt and gets fatter towards one end - that looks like the issue from the picture at least.

Two "possible" reasons. Firstly a die will not cut a perfect thread all the way up to a shoulder as it has a lead in. Your bolt looks good though for the major part. Did you use a parallel finishing tap to make sure you have a perfectly formed thread all the way through the drilled hole?
The screw was to replace a missing one in my 4 way toolpost.
The ones in there never want to unscrew all the way but I found one that did.
It ran down the other hole which I cleaned up with a tap fine.
The screw I made won't go in either hole properly.
I just tried the die on the other screw. Gets on a little way then it's trying to cut so I stopped.
I suspect it is a duff die .
Non adjustable.

The square head, dog point screws in toolposts have a hard life. Very often, the dog end is bulged as it has deformed under the pressure, which is why removal is often difficult.
If you had many to do, a solution might be to replace with 1/2" UNC dog point grub screws. Pick the correct length for the tool and the heads are flush with the top of the holder, giving one less thing to bang your knuckles on or for swarf to wrap around.
Please do not shoot me for mentioning this, but is the die 100% 1/2" UNC and not 1/2" BSW wearing a stetson to look all modern?

that kinda finish can be found when making a new thread if the die is the wrong way raround....
the tooling securing bolts....I just gentley file the last couple of threads to remove the bulge in the bolt...then they will come out...

Would it not be BSW anyway being a50/60s colchester?
