The driver bit in your pic @spencer 427 looks to be rounded off from a slip
Now before you go down the route of judgement based on this were you using it in the correct sizes Phillips screw. Even the very best of drivers will do this on a forceful turn if you use a Phillips driver in a pozi drive screw or a screw that's way to big or small for it. If it was used with the proper screw then yes that's a crap tool but if it was used on a pozi drive then there is every chance it's gonna slip if it's tight and give your driver edges a hard time.
