looks like a bespoke powerpack for a specific application, A look on Sara's website shows some hydraulic loading bay levellers and scissor lifts.
Although the power pack is rated at 210 bar and 3.7 lpm, I suspect the applications are not in constant use. A fully enclosed motor and pump without a cooling system would not be suitable for a continuous use application.
Likely the ports are just an internal manifold to allow multiple connections, perhaps to two cylinders - easy to check with an air line.

Looking at the SWL on the item, it is likely off a loading bay/dock leveller. As above, it would low duty cycle - to level up the dock to the lorry that's just pulled up and maybe occasional tweaks as the lory rises or falls as the goods are loaded or unloaded.
