I think that may be it.Does look a bit dirty but I would check the internal thread as it goes past the outlet of the oiler. Could have picked up a thin sliver of thread from that or even a build up of dirt.
Mind you don't cut your finger!

Fascinating, it was on my list of things to google, honest.Yikes. Had never heard of thread galling but this is an excellent description of what happens and how to (try to!) avoid it.
Glad this isn't what you were faced with, Fizzy!
Bolt Depot - About Thread Galling
Could do. This Old Tony did something similar I think.Yesterday I scraped out an internal fine thread of 20mm with an M6 tap wich had same pitch as an alternative to a thread file, worked pretty well. Migth help here maybe?
Must confess; think I saw it there.Could do. This Old Tony did something similar I think.
Please don't @fizzyAnd had a really whacky idea. Hopefully not necessary but I wonder if this would work?
If I needed to rectify the thread inside the tailstock I could try this.
Thread cutting boring bar in a 4 jaw.
Reverse the tailstock.
Place tailstock the wrong side of the saddle.
Push it along with the carriage to cut the thread.
Very last resort.Please don't @fizzy
The tap in your hand is a better idea. Get an inspection glass and look at each thread carefully.
Usually the tailstock will run on a V and a flat so you will not be able to reverse it.And had a really whacky idea. Hopefully not necessary but I wonder if this would work?

I think that is just the camera flash but will have a look.I would check out why there appears to be a chunk of metal recently chipped from the first thread (Shiny nick, top left of the thread).
It looks to me that the whole unit may have needed better preparation (cleaning) before re-assembly?
