Also worth mentioning...if the offending item is a none re-settable type (which it very much looks like to me), they have a limited life. They will only withstand a certain number of 'minor' flashbacks & then have to be binned & replaced; there is also no way of annually testing none re-settable arrestors (as required by HSE) to check they are functioning corretly, which you can do with re-settable type arrestors.
Could well be that said compay was skimping on equipment...hence no reply from the supplier

I was trying to suss that out looking at the photograph Wozza posted. They don't even look like re-settables to me, just standard none re-set inlines; that badly burnt though it's difficult to place em. Weldability own brand some decent stuff but I'm not familiar with that model mentioned.
Flashback Arrestors have to pass rigourous safety tests (in Germany) & I wouldn't touch any that are not British/European/American made.

Please be aware that the incident report to which Wozzaaah has linked above was found to be incorrect following a full investigation by Balfour Beatty Carillion (BBCJV). Weldability flashback arrestors were found not to have been involved in the fire at the East London Line Project site after all, which is not surprising as Weldability’s flashback arrestors are manufactured in Germany to full EN730-1 compliance and are 100% guaranteed.

You can read the BBCJV retraction and confirmation of the equipment actually involved, in their Safety Sheet No 136 here. You will note that BBCJV have confirmed in their report that the flashback arrestors found to have failed were SWP brand products. An independent test of the SWP flashback arrestors under EN730-1 conditions has been carried out and you can watch a video of these tests here.

Hi , Brian thank you very much for passing this info. personally I have known SWP tools and accessories for a long period of time and never had any problem,I have been on the trade for about 20 years and if all the rules are respected in relation to the health and safety [ like verifying all the hoses and connectors on the bottles, flash back arrestores and torches for leeks and malfunctions, and disconnect the hoses from the bottles in the end of every shift and discharge the gas left in the hoses ]there is less probabilities that an accident like this can happen

I have just fitted some new to me pipes to my oxy/accet bottles and they have inline type flashback arrestors...
