    Someones in for a slapped hand.

    Looks like the needles haven't been mixed throughly enough. I believe they are added them once one site.

    Looking at the partly washed aggregate, I think the last truck has washed out the shoot, the next hasn't mixed the reinforcement in enough, so it all came out at the start covering the top layer of mesh. Stopping any concrete falling through; causing the void.

    Anyone seen this before.

    Why use steel fibre reinforced concrete and a seperate steel mesh?

    Use one or the other.

    Steel fibre should be mixed in at the batching plant before it goes into the truck for transfer to the site.
    Same for plastic fibre, unless a blowing unit is used on site to at them at the last minute.

    Sorry It was conjecture they were added on site. Its a long long time since I had anything to do with batching concrete. The last time I was a round it, it was added on site.
