Just a thought, do the brakes get unusually hot at all?
After youve checked everything on the engine, it might be worth checking everything spins freely and nothing is binding, theres oil in the transfer box and diffs, etc. When I first bought my rangie, it had no oil in the xfer or axles (or the wheel swivels), and adding the right amount of oil at the correct grade to these picked me up quite a few mpg's...
Im reminded also of when someone borrowed my van a while back, and brought it back moaning that it was gutless and returned sub 10mpg (normally it gets about 20 which isnt bad for something that you could put your average car inside the back of). Turns out they forgot to take off the (admittedly at the time, a bit weak) handbrake, not realizing it was a hockey stick type on the steering column (thought it was electrical apparently). Of course all the handbrake mechanism had to be replaced where it had been burnt away during the journey. Saved me having to chock the wheels up on steep hills anyway as then I HAD to fix it. I assume my friend either has no sense of smell, or goes around expecting vans to smell of burning brake linings. But there are people like this in the world!
Also one last daft thought, are they driving it round in 4wd permanently? when I had a shogun the 4wd wasn't permanent, it required a lever to be thrown inside to engage the drive to the other axle, on the later models they still do this somehow but its engaged by electrical means I think, could be that somethings up electrically and its perm selected 4wd which will mess up your consumption.
