Is it possible that the shroud is shorted to the tip, and the shroud is then contacting the puddle or the work? That would put welding current directly through the shroud, and I would think it could cause this type of damage in a few seconds. Just a thought...
I've always had some this problem to one degree or another. I found that either scratching a straight line with a scribe, or using an angle grinder to put a very shallow "V" at the joint makes it easier for me to follow the joint.
